When scheduling appointments please include all insurance information with name of the insurance company and members ID # so that we can pre-authorize your visit and check your benefits before you arrive. This will save you time. You can also now fill out an online information form found on the office forms page.

Please note that no appointments will be honored at 1PM or 1:30PM. Please include all insurance information when booking your appointment.
